In which Quebec emergency rooms are patients most likely to return home without having seen a doctor? The “2020 Ranking of ER Overcrowding in Quebec” describes the situation over the past year and measures the evolution of the situation over five years using data obtained through a request for access to information, before the COVID-19 pandemic.
Our health-care system is unable to meet the demands on it — and the effects of population aging are only beginning to be felt.

Il est temps d’aller au-delà du faux dilemme entre l’universalité et le modèle américain, et de regarder vers l’Europe
By one estimate, at any given moment, over one million Canadians — three per cent of the entire population — are waiting for a medical treatment.
Despite decades of animated public debates and the colossal sums spent—which keep on increasing—waiting times for Canadian patients continue to worsen, notes this study published by the MEI.
The government could give the health care system a real boost by getting rid of med school quotas.

Facilities funded by the public purse but run by entrepreneurs provide a better environment for patients.
What does entrepreneurship contribute to health care? Despite the example of Europe, where companies have long had a large role to play within universal systems, simply posing this question in Canada can elicit strong reactions. And even when European successes are mentioned, other reasons are always suggested to explain their better results.

The technologies behind telemedicine are available, and more Canadians could receive remote care, at home or at work, thereby saving themselves trips and avoiding long waits. Unfortunately, our governments are slow to remove obstacles that prevent patients from benefiting from virtual consultations, and doctors from providing this care.

Waiting times in Quebec emergency rooms continue to increase.
Earlier this year, the media reported that the situation in Quebec emergency rooms had improved in 2018-2019. Yet when we take a closer look at the Department of Health and Social Services data, we see that the opposite has happened, despite a reduction in the number of patient visits.

Press freedom is closely linked with the degree of economic freedom that prevails in a country.
The latest reports on freedom of the press are worrisome. This freedom has declined over the past decade, and 54 journalists were killed on the job in 2018 alone. Several factors explain the level of press freedom of a given country: the nature of the political system, its level of development, or the presence of armed conflicts on its territory, for example. Another important factor, too often ignored, is its degree of economic freedom.

Quebec’s Health Minister recently announced that she wanted specialized nurse practitioners (SNPs) to be able to make diagnoses, as is the case everywhere else in Canada. The Collège des médecins du Québec (CMQ) ended up making peace with the idea, while the Fédération des médecins omnipraticiens du Québec (FMOQ) is still not on board, claiming that this act must be reserved to physicians. Emergency room wait times are stagnating in Quebec.
“Hospitals: Quebec brings order to emergency rooms — No patient will stay more than 48 hours,” ran a front-page headline in La Presse on March 11… 1980! Last year, the minister of health repeated this ultimatum. Waiting times at emergency rooms have been making news for a long time. Data released this past spring suggest that the situation has improved and that waiting times have declined, especially for patients on stretchers. What is actually going on?

The more economically free a country is, the more press freedom exists there.
The virtues of press freedom are widely recognized today. Economic freedom, another essential liberty, is for its part underappreciated. Yet it makes a substantial contribution to the improvement of human well-being, in addition to which it is a necessary condition for ensuring a certain degree of press freedom.

Entrepreneurial dynamism meets the demand for care for the elderly.
It seems that each week brings its share of bad news about the Quebec health care system, to the point that we forget that certain of its components work rather well. This is the case for senior housing and care, largely provided by the private sector.

Government control over access to medical training hurts patients.
Quebec’s Health Minister announced a reduction in the number of medical school admissions last year in order to keep doctors from ending up unemployed in the future. And yet, one in five Quebecers still does not have a family doctor, and proportionally, Quebec has fewer doctors than most industrialized countries. The problem with health care and education is not the lack of resources.
In November 2017, Quebec announced a $1.1-billion personal income tax reduction. Some argued then that the reason the government could afford to reduce the tax burden was that it had reduced spending on health and education, and that these sectors had been starved by years of successive cuts. However, both health care spending and education spending have increased considerably in recent years.

The Canadian federation was founded in 1867 in part to ensure a common, unified market across the country. This ideal is enshrined in Section 121 of the Constitution. Yet while substantial trade does take place between the provinces, the dream of a truly unified market remains elusive. An important case about to be heard by the Supreme Court of Canada, though, may help bring down the barriers the provinces have erected to restrict trade over the years.

Quebec’s Health Minister recently gave an ultimatum to the province’s hospitals such that emergency room stays could no longer exceed 24 hours. While our health system has failed for years to significantly reduce wait times, the performance of a Swedish hospital (the Saint Göran, a Stockholm hospital funded by the government and run by Capio, a private multinational company) should inspire decision-makers within our health care system.
